One of the most important safety strategies is to have an up to date medication list.
Your healthcare team needs to know what medications you take and how you actually use them. It is critical for helping to make good decisions for your health.
LET’S REVIEW WHAT YOU SHOULD HAVE ON YOUR LIST:
- Your name, date of birth, family doctor and pharmacy information
- Allergies or intolerances (these are medications you don’t want to ever take again)
- All prescription mediations and non-prescription medication you take. Include all daily pills and:
- weekly and monthly medications
- inhalers
- eye/ear drops
- patches
- sprays (mouth or nasal)
- rectal, vaginal and other medicated creams
- injections or infusions from clinics
- medications you take “as needed”
- date the list is made
Set a reminder in your phone or on your calendar to review and update your list at least every 3 months. Also, make it a priority to update it after doctor’s appointments or hospital visits.
